Cloudkill and Exhale have changed.

The new formulas should be less buggy and fix a few abuses, but may still
require some minor twinkage.

In general the effect of the change is a lack of stacking, but more
consistant long-term damage and a fix of a few abuses.

Hydras in particular should be more consistent with their damage.
