i've changed the majority of the skills/prayers/items that use the
death_hook code.

a side effect that most invulners wont like is this:
if you have more than one effect that would prevent you from dying
(invuln, martyr, bless, rez amulet, etc), then the order in which they are
applied is arbitrary, essentially random.

so: if your troll wears a rez amulet, he might have it pop before invuln
is checked... or he might not. luck of the draw.

that's the way it's gonna stay for now. i apologize to anyone this hurt -
i'm not really trying to reduce you, i'm justtrying to clean up a badly
hacked-together section of code.
