Whilst researching the waking dream, scolars and clergy have
discovered another function of the hearthstone relics. For a
brief period during which the stone is cooling down from use
the stone retains a dwindling, single link to the location from
which it was previously used. Focusing on this link allows the
stonebearer to return to where they had left in those fleeting
moments prior. As the stone disperses the extra energy, the link
grows weaker and eventually severs itself.

tl;dr
hearth return will return you to the last place you hearthed _from_ while the hearthstone is on cooldwn for a fee. This only works once per cooldown and resets when you trigger a longer cooldown.
